... If anything happens with AAs we are the ones thrown under the bus. I say again... Why on earth would I supervise an AA?!?!!?!!
Seen by PA & "sent home with two medications" "supervising consultant, who the coroner found never reviewed the patient in person, agreed with the diagnosis."
The 25 year old had an aortic dissection & died.
Not enough supervision is happening.
Tragic.
"It is a matter of concern that despite the patient’s reported symptoms, in view of his age and extensive family history of cardiac problems, Mr Peters was discharged from the Ambulatory Care Unit without being examined / reviewed in person by a doctor."
"It is a further matter of concern that (according to the evidence of [REDACTED] , Consultant Physician) no policy or protocol exists within the Trust as to when patients may or may not be discharged from the Ambulatory Care Unit without a medical review taking place."
Response from trust:
"In all these circumstances it is the professional responsibility of the supervising consultant to ensure that they have confidence in the information provided by the practitioner ... before reaching a clinical decision."
