@allisonoconn Disagree. It’s actually the best way to learn anatomy. And get some dissection skills. I honestly can’t think of an alternative. And every “dead human body” was donated with full understanding of what we would be doing. Gross anatomy is still a good thing for medical students.

Heart failure is more deadly than most cancer diagnoses. Let’s treat this disease with greater respect. Putting heart failure into remission means giving the disease our best fight. Carvedilol 6.25 and losartan 25 and see you next year is not our best. I’m here all day.

I saw 26 patients suffering from heart failure on consult rounds yesterday. Truthfully, looking back on the day, no patient got my very best. Because having so little time with each patient is never the best. I am particularly haunted by the patients for whom prognosis was heavy.

Never forget, coronary arteries fill during diastole. The gradient is defined by 1. Diastolic AoP at the “top of the hill” and 2. LV diastolic pressures at the “bottom of the hill” A patient with low MAP and very high LVDP / PW may have coronary ischemia. Unload the LV.

I’m saying this louder for the people in the back. SGLT2i are NOT nephrotoxic! They are actually kidney protective! Watch the GFR yes but don’t just reflexively punt the kidney saving medication because the creatinine bumped a little (as expected).

A patient today refused to wear his mask over mouth and nose in our waiting area, claiming the mask wasn’t necessary, despite our clear policy otherwise. I went out to the waiting area and demanded he leave. He did. This is a place to get healthcare, #respect the care team.

I now routinely work to start SGLT2i and MRA simultaneously for patients with heart failure. Ejection fraction independent. These are complementary therapies with solid evidence for all patients with heart failure. Using them together helps mitigate hyperkalemia risk.

VA-ECMO by itself does NOT “help the LV” in myocardial infarction. It loads the LV especially when running the speeds high. High preload, high afterload. Increased VT/VF and increased pulmonary venous pressures. The VA ECMO, without a drain, is to save the patient, not the LV.

Today my patient who used to have a BMI of 50, now down to 25 after gastric sleeve and total overhaul of lifestyle including routine exercise, officially came off diuretics, anti-HTN meds, and even metformin (after coming off insulin). I was just #grateful to be her cheerleader.

Our system will promptly pay for an ICD after 3 months of homeopathic dose generic ACEI but won’t pay for the legit dose of ARNi that can reverse remodel the LV to the point of not needing ICD at all. Insanity.

6 months ago, I saw this patient, 30ish woman, with acute HFrEF in my clinic. EF 25% Started sac/val, furosemide initially. Brought back quickly, and then added MRA/SGLT2i. Brought back another month later, added carvedilol. APP titrate all to nearly max. Bets on EF?
