If memory serves, I believe a theorised origin of "Lef" is old french. The English started using it before the french later changed to "Lieu." At that point, the English refused to change.

Historically speaking. Back in the day Officers were shoulder boards to identify themselves. Lieutenants were gold on the left side only.
Hence Leftenants.
