One of my HO friend actually quit HO. He barely in 2nd posting. He thrived well in first posting but in 2nd posting cant barely tolerate 2 weeks because toxic verbal abuse from MOs. And when he tried to seek for help, even Psy team dont bother much to actually “elevate” him.

Got a nice case yesterday. 57 years old gentleman came in c/o sudden weakness of all limbs after pergi mengurut. Not sure how bad or intense the massage was but he denies any excruciating pain during the session. But claimed after that unable to walk and been bedbound past 2 week

Bila KKM nak improvise daily attire in Gov Hospitals? I believe working with using scrubs is much comfortable than wearing formals. Medicos in overseas wear scrubs as routine. And with labcoats outside if counselling patients in clinic.

Okay as Medical MO, I never force them to stay back and do work. In fact I always enforce them to go back as sharp as possible. And I even call the PM or oncall HO to come and clerk the new referrals. Normalize kerja balik on time!! Normalize balik on time is not a pemalas!!

This is an honest opinion to all specialists, surgeons, medical officers. To Us. If you see your HO still in ward after their shifts are over, let them go back. Ask them to go back. Why do you have to force them to stay? They are not paid to stay longer than that.

During my oncall yesterday, saw an admission for CVA. Reclerk and attend the case to find out that it was not a simple CVA. Apparently patient had Right UL pain for almost 1/52 and worsening for the past few days. Pain was described as “cramping/ numbness” worsen with movement.

Sedih ya jadi kontrak doktor ni. Masa dalam service, kerja kudrat diperah keringat ketitisan terakhir tapi diperlekehkan. Gaji tak ckup, further study xleh. Dah quit pergi GP, kene tipu dgn GP. offer gaji 4-6K je. Korang tau tak gaji GP 2-3 tahun lepas adalah 10-12K ya sebulan.

I have had a patient who came in ED with lower limb weakness. He’s pretty young, 28 years old. He denies fever, loa even other infective symptoms. The weakness is rather insidious in past 1-2 weeks. Denies any trauma/fall too.

20-30cc/kg ya. Bukan 1 pint over 1 hour lepas tu tak improve terus inotropes. Primary care and Emergency department need to take heeds. Inotropes with depleted intravascular volume will only worsens tissue perfusion.

Alhamdulillah. So altogether, I passed my MRCPUK in 1 year and a half. Took my part 1 in May 2018 My Part 2A in October 2018 And passed my PACES with single attempt (syukurrr sangat) in October 2019. If you put your effort and tawakal in, you can! Insha Allah.

1. Another amazing case encountered while oncall yesterday. Middle age man, came for pain in the left lower limb. Turns out its acute limb ischemia, again. Our vascular team proceeded with left femoral embolectomy. Turns out despite embolectomy, he has persistent pain.

Any female patients in their reproductive age group who came with lower abdominal pain should suspect ectopic pregnancy. That’s why it is our procedure and it is wrong for any medical expert not to send UPT to exclude. Pesakit harus faham our diagnosis is based on exclusion too.
