Even without the slur, his reaction to her having an accident was to get angry with her. After three years why doesn't he trust her and give her the benefit of the doubt?
He can be mad it happened but why mad at her?
This is sunken place territory. That man has absolutely called her that before, either in his head or around friends & family. His friends & family have probably called her that. And if it happens in front of him, he’ll probably join in on the taunting or say it’s not a big deal.

Life gets hard, people get overwhelmed and react poorly. What they don't do, is call their girlfriend the hard R. He's right, he slipped. Meaning he thinks it all the time and says it when she's not around and now he slipped up revealing his real self.
